Expression of Interest

Expressions of Interest should be emailed to DataServices@health.wa.gov.au as Word attachments. Please submit the Application for Data form, relevant data services forms and variable lists and a research protocol.

Formal Applications

Application for Data where DOHWA HREC approval is required

Please submit 1 original and 12 copies of the following

  • Application for Data form
  • Data services forms
  • Variable lists
  • Any supporting documentation (e.g. participation information sheets, consent forms & information about data collections required outside of DOHWADepartment of Health Western Australia)
  • DOHWA HREC Application for Ethical Review (Please see the HREC website for this form)
  • Detailed research proposal/protocol (this is in addition to the brief outline included in section 3 of the Application for Data form).
  • Any documentation regarding approval from other Human Research Ethics Committees

Please also provide 1 copy of the following:

  • Confidentiality agreement/acknowledgement for all personnel listed on the project (either for Researchers or WA public sector employees).

    • Please ensure the Indemnity forms (last page of the Application for Data form) have been read, understood and signed by the authorised institutional officer.

    Application Checklist

  • Signed Confidentiality Agreement/Acknowledgement for all personnel listed on the project (either for Researchers or WA public sector employees)
