See also http://ww2.health.wa.gov.au/Corporate/Articles/N_R/Research-ethics has approved the project, the final application must be lodged with the DLBData Linkage Branch: the specialist team at the Department of Health who are responsible for developing and maintaining the WA Data Linkage System, performing data linkage, and the facilitation of access to linked data.. This step is required in case there are any changes required to the application as part of the ethical review process, after in principle support has already been given. Please submit electronic copies of the final versions of the documents below to DataServices@health.wa.gov.au. It is recommended that the package of documents is zipped to save space.

  • Participant Information and Consent Information (if applicable)
